Would this give you a clear development and test process that yields predictable results?

Some sort of formal QA process including:


- Requirements analysis


- White box testing focusing on saftety critical items


- Black box testing and system testing done by an independent group using real-world scenarios.





How come a lot of this gets dropped in real-world projects ? Only explanation must be the projects are populated by phonies.Would this give you a clear development and test process that yields predictable results?
